Workplace Harassment, Bullying and Discrimination Risk Assessment

This fully completed workplace harassment, bullying and discrimination risk assessment example has been prepared by a qualified health and safety professional. It covers prevention and management of harassment, bullying and discrimination in the workplace, and can be used as a starting point when developing your own risk assessment documentation.

What Does This Workplace Harassment Risk Assessment Cover?

This risk assessment identifies the key hazards associated with workplace harassment, bullying and discrimination including;
  • Verbal and psychological harassment — unwanted abuse, intimidation and social exclusion causing significant psychological harm, stress and depression
  • Sexual harassment — unwanted conduct of a sexual nature causing significant distress and psychological harm
  • Discrimination based on protected characteristics — less favourable treatment related to age, disability, race, religion, sex or sexual orientation
  • Failure to support victims — inadequate response to reports of harassment causing secondary harm and constructive dismissal risk

Each hazard includes an inherent risk rating ranging from Moderate to Substantial, and a comprehensive listing of applicable control measures covering areas such as;

  • Anti-harassment and dignity at work policy communicated to all staff and enforced consistently
  • All staff trained in acceptable workplace behaviour with managers trained in recognition and early intervention
  • Confidential reporting mechanism with multiple routes including HR, independent nominated person and external helpline
  • Sexual harassment specifically addressed with third-party harassment from customers and contractors covered
  • Equality and diversity training with recruitment and reward processes reviewed for bias
  • Employee Assistance Programme and occupational health available to all who report harassment

A residual risk rating is then assigned to each hazard, demonstrating how effective risk management reduces overall risk to a tolerable level.

A Professional, Easy-to-Use Risk Assessment Template

This document uses colour-coded header styles matched to risk ratings, making it quick to read and easy to communicate during toolbox talks and worker training. A built-in risk rating matrix supports consistent evaluation of likelihood and consequence, and hazard pictograms using internationally recognised ISO warning symbols are included alongside relevant hazard entries.
The result is a completed risk assessment that looks professional, reads clearly, and demonstrates a structured and effective approach to risk management.

Who Is This Risk Assessment Template Suitable For?

This workplace harassment risk assessment template is suitable for health and safety advisers, HR managers and any employer with a duty to assess psychosocial risks under the Management of Health and Safety at Work Regulations 1999, the Equality Act 2010 and the Worker Protection (Amendment of Equality Act 2010) Act 2023. It can be implemented as a standalone document or as part of a broader occupational health and safety management system aligned with ISO 45001.

Note: This example risk assessment is supplied for use as guidance only and should be reviewed and completed by a competent person.
